HIPAA

Formlabs is not a Business Associate for HIPAA purposes as we do not require nor desire access to Protected Health Information (PHI). Formlabs requests and recommends that users do not include PHI in any file names. We cannot take liability for any PHI sent to us and advise that all users of our technology are familiar with de-identification methods and de-identification implementation specifications. Formlabs cannot provide guidance with respect to HIPAA compliance.

Further, if users utilize our optional, cloud-based Dashboard tool, a user may contact Formlabs Support to permanently delete file names from our Dashboard records.

More details about Dashboard can be found in the support article Dashboard overview and setup. There may be additional IT and security considerations for your institution when evaluating cloud-based tools.

For more information, please review the Formlabs Privacy Policy, including the Data Collection section, here: https://formlabs.com/privacy-policy/.

Data retention on hardware

The Form 2, Form 3/Form 3+/Form 3B/Form 3B+, Form 3L/Form 3BL, Form 4, Form 4L, Fuse 1, Fuse 1+ 30W, and Fuse Sift have two firmware images, with all print-related information stored on the user-accessible image. This image is completely wiped during a factory reset, which any user can initiate and complete on-premises. Any information you upload to the printer (e.g. file names of models that are sent to the printer) would be erased with a factory reset. Instructions for a factory reset can be found at the bottom of the relevant error message support articles:

The other firmware image does not receive any user or print-specific data and cannot be modified by users.

If there is a particularly sensitive file or a file with sensitive information (e.g. PHI) is accidentally uploaded, one can always manually delete the specific file from the printer without a factory reset. To delete individual prints on a Formlabs SLA printer, go into the Jobs > History, select the individual job name, and select Delete. To delete all prints on the machine, open the Settings menu, then select Storage.
